Support at Home
Support at Home is a government-subsidised program under My Aged Care. It used to be known as Home Care Package. The program helps people over the of 65 to live independently at their own home for as long as they wish
Australian Nursing & Training Services (ANTS) is an approved Support at Home program provider by the Commonwealth Department of Health to support clients to have a quality life at their own homes with assistance.
Services provided is based on Consumer Directed Care (CDC) principles where clients need to get actively involved in the service design and delivery processes.
ANTS commits to provide highest quality services to its clients.
Program for your care
- Level 1 – for people with basic care needs
- Level 2 – for people with low-level care needs
- Level 3 – for people with intermediate care needs
- Level 4 – for people with high-level care needs
Support at Home is designed to help you live well and stay in your own home for as long as possible.
We offer a wide range of services that make day-to-day life easier and support your health, independence, and wellbeing.
After your aged care assessment, you’ll be matched with a list of approved services based on your needs and budget.
You’ll have the flexibility to choose the combination of services that work best for you – and you can change them anytime as your needs evolve.
Your provider will work with you to make sure you’re always getting the right kind of support.
What kind of help is available?
You can choose from a variety of supports, including:
- Health and clinical care – nursing, occupational therapy, continence support, and other clinical services.
- Independence support – assistance with getting dressed, taking medication, travelling to appointments, or taking a short break with respite care.
- Everyday living – help around the home with cleaning, gardening, shopping, or preparing meals.
Ongoing and short-term support
Support at Home has eight ongoing service levels, which determine your funding based on your care needs.Each level gives you a quarterly budget to spend on your approved services. If your needs change, you can ask for a new assessment at any time.
If you already receive a Home Care Package, you’ll automatically move to Support at Home with a similar funding level.
There are also three short-term service options for people who need extra help for a limited time.
Your aged care assessor will let you know if you’re eligible for one of these short-term pathways.
How to enter the Support at Home program
You can do this online at My Aged Care, or by calling 1800 200 422.
If your application is successful, you will be referred for an assessment. Assessments are done in person, at your own home.

How to get Support at Home services
Follow these five easy steps to get your My Aged Care Support at Home Program.

Call our Intake Officers on 0410928269 or email us.

Assessment
You will be assessed by the Aged Care Assessment Team (ACAT) in your home to determine your eligibility for a Support at Home program and level of care. If you wish, we can be with you for this assessment.

Waiting list
You will receive a letter from My Aged Care informing you whether you have been approved for funding, and at what level. Once approved, you may be put on the national waiting list for a Support at Home program depending on availability at the time. Contact us if you need assistance while you’re on the waiting list.

Allocation
You will receive a letter from My Aged Care when you have been allocated a Support at Home program. If you’d like Advance Diversity Services to be your provider call us on (02) 9136 4788. We’ll discuss the types of services you wish to receive, and how these services will meet your needs. We offer no-obligation consultation.

Sign The Agreement
Sign your service agreement with us and your services will commence.
Australian Nursing & Training Services also provides its services to the clients who do not have a Support at Home program, and fee for services arrangement can be made separately.
