This skill set reflects the skill requirements for case management work in the health or community sector.
This skill set is for Priority qualification, anyone in the Aged care and Disability industry and the student must:
- Be 15 years of age or older
- Must no longer be at school
- An Australian citizen, permanent resident holder or New Zealand Citizen
- Living or working in NSW or an aboriginal or Torres strait Islander student who does not live or work in NSW but lives in specific defined interstate NSW border
- Must have a valid Unique Student Identifier (USI).

- CHCCSM004 Coordinate complex case requirements
- CHCCSM005 Develop, facilitate and review all aspects of case management
- CHCCCS004 Assess co-existing needs
- CHCCSM006 Provide case management supervision
These units provide credit towards a range of qualifications in the CHC Community Services Training Package.
Statement of Attainment
You will be able to obtain a statement of Attainment after successful competition of this unit of competency from the CHC Community Services Training Package meeting industry requirements for case management work in various health or community services contexts.
